Once your relationship with your npc gets to a certain point they will get headwear that you can have them put on or take off at the rendezvous point. After that after a few quests you will get additional headwear to switch around on them. Not sure if they headwear gives them more defense or not.

This is the list of headwear we currently know of that your npc can get to wear.
