This is also known as the Optical Hat quest.

You must gather five eyes from slime NM across Vana'diel.

Burning Hakutaku Eye from Thousand Eyes (Lvl 60-62) in King Ranperre's Tomb
Damp Hakutaku Eye from Dodomeki (Lvl 63-69)in Ifrit's Cauldron
Earthen Hakutaku Eye from Blubber Eyes (Lvl 55-58) in Sea Serpent Grotto
Golden Hakutaku Eye from Mindgazer (Lvl 66-69) in Sea Serpent Grotto
Wooden Hakutaku Eye from Million Eyes (Lvl 73-76) in Rancor's Den

Once you have all the eyes you will need to find an Adept Alchmist (around level 84) to create for you an Hakutaku Eye Cluster.

1x Light Crystal
1x Burning Hakutaku Eye
1x Damp Hakutaku Eye
1x Wooden Hakutaku Eye
1x Earthen Hakutaku Eye
1x Golden Hakutaku Eye
1x Vial of Fiend Blood

Then trade the Hakutaku Eye Cluster to a ??? in the Den of Rancor to spawn the legendary NM Hakutaku, which drops the Optical Hat (100% drop).
